Ask what a word or short form means in its full context.
A health record may use a short form for a longer phrase. The same short form can have more than one meaning.
Keep the full record for the care team. A line taken out of context may leave out a key fact. Removing your name does not make it anonymous.
ChatGPT can help you phrase a question. It should not guess which meaning applies to your chart.

- Ask the clinic to spell it out and explain it.
- Check whether it calls for a next step.
- Keep the clinic's answer with the original record.
