How do I write down my symptoms so my doctor understands?
Describe what happens, when it happens and how it affects you.
Your own words are a good start. Write where you feel the symptom, when it began, how long it lasts and how often it happens.
Note what seems to make it better or worse. Say if it stops you sleeping, eating, walking or doing other usual tasks.
Keep exact dates, readings and units in your private notes. If you are unsure, say so. You do not need AI to make your symptoms sound more medical.
- Write the main symptom in your own words.
- Add when it began and whether it is changing.
- Record how it affects your day.
- List medicines or other steps you have tried for it.
- Give the full note to your care team through their usual route.
Make an empty symptom-note form. Include what I feel, where, when it began, how often, changes, effect on my day and what I have tried. Do not suggest a cause.
Paste the message into ChatGPT, then fill in any blanks before you send it. ChatGPT opens in a new tab. Opening it does not send the message.

A note is for sharing facts. If you need urgent care, get help rather than waiting to make the note perfect.
